Change of Ownership - IA

Required Documents:

• Letter of instruction signed by the executor, beneficiary, administrator, personal representative or other court appointed authorized signer, including capacity with a Medallion Signature Guarantee

OR

• Change of Registration form with MSG (529 accounts only)

• Completed New Account Application

• Affidavit of Domicile

• Inheritance Tax Waiver (where applicable by State requirements)

Delivery Methods:

Additional Information:

• Letter of instruction needs to include:
  • Fund and Account number
  • Signer's capacity should be included
  • Name of deceased and or name of estate
  • Detailed instructions what needs to be done
  • Date of death

Required Documents:

• Completed new Account application with MSG signed by all owners with letter or instructions

OR

• Change of registration form with MSG for both owners and W9 for new owner being added

• If W9 is used in place for Change of registration form then new owner's Date of birth is also needed.

• If new account application has an MSG, letter of instruction does not need MSG
